Instructions for Submitting Candidates

Please submit separate electronic nomination packages for each award nomination, even if the same individual is nominated for more than one award. Please submit the nomination form(s) and dossier(s) to Janice Mackovitch at

2015 American Epilepsy Society Awards

Instructions for nominating candidates for the William G. Lennox Award, the AES Service Award, and the J. Kiffin Penry Award:

Candidates are formally nominated by one of their peers. It is the responsibility of the proposer to ensure that all required documents are obtained and that the complete candidate dossier is received in the AES Executive Office by the appropriate deadline. Strong preference is given to members of the Society. A complete proposal consists of the following:

  1. Completed Nomination Cover Sheet— Be sure to indicate which award the nomination is for.
  2. A summary (not to exceed two pages) explaining why this candidate deserves this award — This should be a descriptive overview and address the award purpose as stated in the award description.
  3. One letter of recommendation is recommended.
  4. Single electronic file. 

These awards will be presented at the AES Annual Meeting in Philadelphia, PA, December 2015.
