Seizure Semiology of Temporal Lobe Origin in Children of Different Age Groups

RATIONALE: To evaluate the value of seizure semiology in children of different age groups with temporal lobe epilepsy (TLE) considered for epilepsy surgery. METHODS: 14 children (2-18, mean 10 years old) - seizure free or significantly improved postoperatively- with TLE are included in this study and divided into 3 subgroups (group A:2-8a,group B:8-12a, group C:12-18a). 79 seizures (A: 35,B: 23,C: 21) were retrospectively reviewed for ictal manifestation such as aura, hypomotrial behavior, automatism, head turning, eye deviation, versive movement, dystonic posturing, disturbance of consciousness and secondarily generalization. RESULTS: In all seizures semiology was similar to TLE known by adults. The symptoms aura, hypomotorial behavior and disturbance of consciousness showed significant (Anova) differences regarding the 3 subgroups. The other clinical signs ocurred in all 3 subgroups with similar frequency. Correct lateralization was possible in 58% for automatism, in 63% for head turning, in 75% for dystonic posturing, in 88% for eye deviation and in 100% for version. CONCLUSIONS: Seizure semiology is an important cornerstone in decision for or against epilepsy surgery, for finding the differential diagnosis, for lateralization of the epileptogenic zone and finally for describing subgroups.
