Rationale: To confirm good preliminary results of surgery for lesional mesial temporal lobe epilepsy performed by ambulatory Magnetoencephalography only. Methods: Six patients (4 males, 2 females) with drug-resistant mesial temporal lobe epilepsy (4 left, 2 right) and more than four complex partial seizures per month were examined by MEG-EEG recordings in slow-sleep stage in an ambulatory setting and without long-term scalp video-EEG. Results: In all patients we found magnetic source images in lesion borders. Selective amygdalo-hippocampoectomy through neuronavigation was performed successfully. Conclusions: Surgery is feasible in cases of lesional mesial temporal lobe epilepsy without admissión for long-term scalp video-EEG, reducing costs and providing greater comfort in view of slow sleep MEG-EEG only.
